We are so excited that your child will be joining us for Summer Camp this year! Below are some clear instructions on what to do now, as well various forms that you need to complete. Please read this very carefully and do everything as instructed!

Final Payment Please attach a check for your full remaining balance for Summer Camp to your Health Form, Parts A & B if you did not choose to have your credit card automatically charged on May 14th. You will find the amount that you owe in the account statement sent to you shortly after you received notification of your child’s acceptance into camp. Anyone failing to meet the May 14th deadline will incur a $20 late payment fee.
Medical Forms
Medical forms can be downloaded below. Please remember that the deadline is May 14th for Parts A & B. Anyone failing to meet the May 14th deadline will incur a $20 late medical form fee. Please do not mail Part C in advance - bring this form to registration along with your child's medication to hand into the Nurse. Please note that the health forms must be mailed to us. We do not accept forms by fax or email.
Cabinmate Requests
You will find a section at the bottom of Health Form Part A where you may list two cabinmates request for your child. Please note that requests must be mutual and forms must be received by the May 14th deadline in order for us to process your request. There is no guarantee, however we will do our best to place each camper with at least one friend of their choosing.
Letter to My Child's Counselor
This optional letter proved to be a huge hit last year! Please fill this out to give your child's counselor and staff member additional information about your child. Please mail it in with your Health Form Parts A & B by May 14th.
Made a mistake?
If you have made a mistake on your application, need to switch sessions, need to cancel, etc. please email us at . All requests for any type of change must be made in writing.

Please print out this form and have it fully completed/signed and returned to us by May 14. St. Christopher requires campers to receive a medical examination by a licensed physician prior to their attending summer camp. If your child has had a physical since August 15, 2008, a copy of the physician’s signed “examination report and release to participate in camp activities” may be used this summer. Attach a copy of the physician’s report & release, complete and sign the 2010 health form, and mail it to: Summer Camp Health Form Parts A & B, 2810 Seabrook Island Rd., Johns Island, SC 29455. If your child needs a physical, please call your doctor ASAP to arrange a visit. Anyone failing to meet the May 14th deadline will incur a late medical form fee. Please only print and complete this if you are planning to send/bring medications to camp with your child. Do not send this form now. Bring it with you to camp in the zip-lock bag with your child’s name on both the bag and the medications.

Please print this optional form and fill it out to give your child's staff member and counselor additional information. Although it is optional, it is a great help for our team to help your children enjoy their camp experience. Please mail this in with your medical forms by May 14th (NOTE: this is a change from last year).

Scholarships are available and our goal is that no one ever misses summer camp for financial reasons. We have set up a scholarship form so that St. Christopher will pay 1/3, your local church will pay 1/3 and the family will contribute 1/3 as well. If you are applying for a scholarship please also make sure that the Scholarship Form is returned fully completed and with a check from your church by May 14.
